Predicting the Future II

The study involves a web-based survey that will take approximately twenty minutes. The survey presents various scenarios and asks you to make predictions or indicate on the basis of the information given. We would like you to complete the whole survey, but you may skip any questions you do not feel comfortable answering. Participant responses will be kept in strict confidence. Please answer the questions honestly. Only investigators will have access to your individual responses. Only summarized data will be presented at meetings or in publications. Your participation in this study is completely voluntary. You are free to withdraw at any time, for whatever reason.

If you choose to participate, you will be entered in a raffle to win 1 of 3 Amazon.com gift certificates, each worth $25. Winners will be selected at random, and no participant can win more than one gift certificate. We’ll ask you to provide your e-mail address so we can contact raffle winners. E-mail addresses will not be associated with any responses to survey questions, nor will e-mail addresses be used for anything other than selecting winners of the raffle. The e-mail addresses we collect will be deleted from our files immediately after the raffle.
