CMHA Ottawa Annual Mental Health Service Awards
In recognition of the commitment and services of individuals, groups, and organizations who have made significant contributions to the mental health movement, consumers of mental health services and the Ottawa community as a whole, the CMHA, Ottawa Branch proposes to present Mental Health Service Awards at its Annual General Meeting.
This year's award(s) will be presented at the Annual General Meeting to be held at noon on Friday, September 28th, 2007, at the RA Centre Clark Hall, 2451 Riverside Drive.
The awards will recognize contributions in one or more of the following categories:
Nominations for the award(s) will be made by two individuals or by a representative of an agency/organization, using a form supplied by the CMHA, Ottawa Branch.
Nominations may be made in the official language of the nominator's choice. Nominations must be received at the CMHA, Ottawa Branch office by 4:00 p.m. Friday, July 6th, 2007.
Selection of honourees(s) will be made from the nominations received, by a panel of persons actively involved in mental health and related services. Membership of the panel will reflect the bilingual nature of the Region.

- Professional or volunteer frontline provider of services to consumers of mental health services;
- Concept and/or delivery of a program of extraordinary effectiveness,
- timeliness and/or innovation;
- Family/friends of people with mental illness;
- Consumers of mental health services;
- Volunteer participation on Boards and Committees involved in the conception, formulation, and realization of policy/programs.
- Preference will be given to nominees whose contributions have not previously been recognized.
- Nominees will be persons/organizations, professional or volunteer, serving the community of Ottawa.
