I recently found through an internet search something called psychotic OCD. I was hoping perhaps someone would have some information or insight into this disease process. How does it manifest and do they often occur together? Is it possible for one to cause the other or is this just something that is completely individualized. Is the psychosis perhaps related to the obsessive thoughts which then become so consuming that they cause a break from reality?

My 19 year old daughter is experiencing psychosis at the present time. Duration to our knowledge has been approximately 2 weeks. She is currently receiving Citalopram 10mg three times daily and Risperdal 0.5mg in am and 3mg in pm. She had admitted to some drug experimentation including hallucinogenics and synthetic marijuana. She admits to having obsessive thoughts that will not stop and is "hearing voices" which include myself and her friends talking to her in her head. She has also had some paranoia which to my observation appears related to the obsessive thoughts as well as the voices. My inquiries are related to her tendency towards OCD. I had taken her to a psychiatrist when she was about 10 yrs old due to excessive hand washing with concerns regarding germs. At the time my husband and I were seperated and I was concerned that the anxiety over our home situation was affecting her adversely. The doctor stated to me after about 5 sessions that if she had OCD it was a very mild case and would probably resolve as she grew older. It did appear to resolve and she later told me that the fear of germs was not related to herself becoming ill but fear that she would make others ill. She has also always had some anxiety issues related to social situations and feeling uncomfortable in new situations or with strangers. I always attributed this to being rather shy and introverted as her father and I both are on some levels. She has also always had a strong tendency to worry even as a small child. I have always called her my "what if" child. She would worry, what if this happens, what if that happens, far in advance of the particulary event even ocurring.

Could she be exhibiting some form of OCD with an underlying psychosis at this time related to her obsessive thoughts? I guess my primary question is, how common is psychosis with OCD or is it off the wall to think they could be related?

I certainly have read a number of different times that there is some overlap in symptoms between OCD and schizophrenia (with schizophrenia being just one cause of psychosis -- not to mention substance abuse and bipolar disorder):

Analysis revealed a general pattern of increasing executive dysfunction in order from patients with OCD and good insight, through to patients with OCD and poor insight, then schizophrenia with OCD, and finally patients with pure schizophrenia.
