So much of OCD diagnosis overlooks the purely cognitive manifestation of thinking too much. Thousand are treated around the world for thinking problems without actually thinking about thinking!

In my experience since 1996 I have seen in the office, and later in SPECT imaging strong evidence that most purely Cognitive Anxiety is not based on deep structures such as basal ganglia and thalamo-limbic imbalances, but rather prefrontal cortex issues associated with executive function - and best treated with stimulant meds rather than SSRIs which can frequently make it worse. These folks don't meet the criteria for ADHD, because Thinking ADHD is not yet on the radar.
